Chylothorax Secondary to Late-Onset Congenital Diaphragmatic Hernia

چکیده
Congenital diaphragmatic hernia (CDH) is a structural diaphragmatic defect that usually presents during the first few hours after birth with severe respiratory distress. However, delayed herniation of the abdominal contents through a CDH may occur beyond neonatal period. In contrast to the high mortality and morbidity rates for neonatal CDH, the prognosis for late-presenting CDH is usually favorable if diagnosed earlier. However, unlike in neonates, the spectrum of manifestations of late-presenting CDH is quite broad and nonspecific. The patients may present with acute or chronic respiratory, gastrointestinal symptoms or be completely asymptomatic [1]. The variety of clinical manifestations for late-onset CDH makes the early diagnosis challenging. Chylothorax, the accumulation of chyle in the pleural space, is a relatively rare cause of pleural effusion in children after neonatal period [2]. Late-presenting CDH and chylothorax are rarely associated. We reported a 3-monthold infant of late-presenting CDH with development of chylous effusion.

High Dose Octreotide for the Treatment of Chylothorax in Three Neonates
Chylothorax is an abnormal condition of lymphatic fluid collection in the pleural space, and the somatostatin analog octreotide is thought to have a beneficial effect on chylothorax. However, the octreotide dosage and administration route for chylothorax have been inconsistent to date. We report three neonatal cases of persistent chylothorax successfully treated with high-dose octreotide infusi...
متن کاملFetal lung and diaphragm malformations
Congenital lung abnormalities are relatively rare but very dangerous for the fetus and neonate. This article describes the most common pulmonary complications observed during pregnancy such as congenital cystic adenomatoid malformation, diaphragmatic hernia, bronchopulmonary sequestration, hydrothorax, chylothorax, lung agenesia, pulmonary hypoplasia. The diagnostic methods and available therap...
متن کاملSilent Tachypnoea in a Neonate: A Rare Presentation of Right Side Bochdalek Hernia with Intrathoracic Kidney
Congenital diaphragmatic hernia (CDH) is a rare condition. The reported incidence of intrathoracic renal ectopia due to CDH is also rare. A right-sided thoracic kidney is much less common due to the location of the liver. Isolated intrathoracic kidney is usually asymptomatic and diagnosed incidentally on chest imaging. The authors report on a 21days old female infant with late-presenting right ...
متن کاملChylothorax after Repair of Congenital Diaphragmatic Hernia in a Neonate: Usefulness of Conservative Management
Aim: Chylothorax is a recognized complication after surgery for congenital diaphragmatic hernia (CDH) in a neonate. Management strategies for chylothorax include cessation of enteral feedings, repeated aspiration, chest drainage, and total parenteral nutrition. It is important to determine which is the better plan for treatment of chylothorax after repair of CDH. The authors report successful m...
متن کاملFive Cases of Congenital Chylothorax Treated by Intrapleural Minocycline
Minocycline pleurodesis was performed on five infants with congenital chylothorax in our institutions. They could not achieve sufficient efficacy though they had received other conservative therapies. Four of the five cases obtained reduction of pleural effusion using the minocycline pleurodesis. We concluded that minocycline pleurodesis is a safe and an effective technique for congenital chylo...
